Patient Profiles

In order to fill a prescription, we must obtain some basic information about the patient for whom the medication was prescribed. Your account may contain as many Patient Profiles as necessary to fill prescriptions for family members, relatives, friends, and even animals. A Patient Profile must include Patient Name, Gender, Date of Birth, and other information the patient may choose to provide regarding allergies or other medical conditions.

Once your Patient Profile has been set up, it can be used to fill multiple prescriptions. Any time you add a prescription to your cart, you will be prompted to "Select a Patient" from a list or "Create a New Patient Profile". For each prescription filled, you will need to verify the information on the "Prescription Review" page. You will also have the option to add a new Doctor or insurance plan depending on the prescription, prior to checking out.

Insurance Profile (optional)

If you choose to use your insurance to pay for your prescription, we will need basic information including; plan name, plan number, plan phone number, etc. Your co-pay will not be included in your total at the time of checkout, however it will be added to your order total following adjudication or verification with your insurance.

Doctor Profile

In order to fill a prescription, we must have the name, city, state, phone and fax number of the doctor that prescribed it. Familymeds will call your doctor to obtain the prescription and verify related information.
